This is an older UIC Class II well in Wetzel county. The permit expired in 2013 and an application for a new permit was submitted but there hadn't been a response to a 2015 notice of application deficiencies letter. The well was considered an active well in May 2016 even though no injection except for a nominal amount in 2011 had taken place.
The most recent MIT took place in 2009. The previous MIT was held in 1997, 12 years earlier.
Major issue
- The well operated without the required MIT for 12 years.
