This is an older UIC Class II well in Wood county. The permit expired in 2001 but it appears that the operator attempted to put the well in service in 2009. A 2010 NOV appears to indicate that the operator was injecting (the NOV cites 47CSR13-13.1.a which has been used by the Office to mean either that illegal injection has taken place or that the operator's permit has expired and a new permit is required). WR-40 reports are not available for this well. The operator attempted to conduct an MIT for the well which failed in 2011. The well was considered an abandoned well in May 2016.

- Injection without a permit. Injection appears to have been taking place in 2010 according to the March NOV.
- Well has been abandoned without being plugged contrary to state law.
