This is an older UIC Class II well in Wyoming county. The permit expires in October 2016 and the operator submitted a permit renewal application in a timely manner, in January 2016. The well has 3 large storage pits for fluid. Three NOVs were issued in June 2015 because of spills on the site. Another NOV was issued in July 2015 because of the well's mechanical integrity failure and improper gauge.
