This is an older UIC Class II well in Ritchie county. The permit expired in 2013 and there has been no attempt by the operator to apply for permit renewal. The well was considered abandoned in May 2016. Google Earth imagery appears to show tanks removed from site by September 2015.

- The well with operated without proper gauges, a permit violation, but no Notification of Violation was issued.
- An inspector found the well injecting at twice the permitted maximum injection pressure; no Notification of Violation was issued.
