This is an older UIC Class II well in Ritchie county. The well's permit expired in November 2013. The operator submitted an incomplete application in October 2013. The operator injected waste in September 2007 after their previous permit expired in August according to a WR-40 they submitted. No further WR-40s were submitted until November 2008 when their permit took effect.
This well is one of a group of wells for which the Office issued an Order in February 2014 allowing them to inject under the conditions of their previous permit. According to the Office's WR-40 database, the operator injected 188 barrels of waste in 2014.
Major issue
- Injection without a permit. Previous permit expired in August 2007. The operator reported injection for September 2007 and filed no more reports until they received a new permit in 2008.
