This is an older UIC Class II well in Roane county. The operator injected waste for years after a previous permit expired in 2004. When their most recent permit expired in 2013 they continued reporting injection through June 2014 after which reporting stopped. The well was considered an active well in May 2016 even though the permit expired in 2013.

Major issues

  • Injection without a permit. Permit expired November 2013, injection took place through June 2014, after which reporting stopped.
  • January 2014 NOV, for an expired permit, operation prohibited without a new permit.
  • May 2014 NOV is an order to cease operations.
  • Injection without a permit. Previous permit expired September 2004, WR40s show injection without a permit until new permit in 2008.
  • Well operated for almost 2 decades since last MIT in 1995.
