This is an older UIC Class II well in Roane county. The permit expired in 2013 and a new application for a permit appears not to have been submitted. The operator has not reported injection from 2011 to the present. When the operator's previous permit expired in 2004 they continued to report injecting waste through 2007. The well's completion report shows no cement behind the surface casing and an inspector in 2011 in an inspection report states that the permit should be canceled and the well plugged. The well was considered abandoned in May 2016.

Major issues

  • Injection without a permit. Permit expired June 2004, injection without a permit was reported on WR-40s through 2007.
  • January 2014 NOV is for expired permit, operation without a permit is prohibited. That NOV was abated and a new NOV was issued in February for the well being abandoned with an additional order to cease operations.
  • The well was allowed to inject waste in spite of the fact there was no cement behind the surface casing.
  • The last MIT was in 1998 and consisted of checking the fluid level in the annulus.
