This is an older UIC Class II well in Roane county. The operator received a new permit to operate in 2016. The operator's permit expired in November 2013 and the operator submitted an application (with deficiencies) for a new permit in January 2014. The Office issued an Order in February 2014 allowing the operator to inject waste under the conditions of the expired permit. The Office issued an Order in January 2015 revoking the 2014 Order and prohibited further injection. The operator reported injection from December 2013 after their permit expired, after the 2015 Order prohibiting injection, well into 2016.

There is a March 21, 2014 NOV for not having an MIT issued to Quaker State which operated the well in the 1990s. The most recent MIT for the well was in March 29, 2014.
