This is an older UIC Class II well in Roane county. The permit expired in 2002. This well was purchased with other wells in 2015 by another operator. The well in 2016 was considered abandoned and the operator was ordered to plug it.

- Injection without a permit. There was a period in 2008, years after the permit expired, where WR-40s by the operator show injection.
